Notification of Address

 Notification of address can be completed at window #7 on the first floor of the Mitaka Municipal Office Main Building or any branch office, in person or by someone who lives in the same household in Mitaka City.
 If a proxy who does not live in the same household completes the notification procedures, a letter giving power of attorney written by the applicant himself/herself or a member of the same household is required, as well as the residence card or special permanent resident certificate—or the foreign resident registration card, which is considered equivalent to these—of the person(s) changing address, and some form of personal identification of the proxy.

Moving into Mitaka City

 If you have moved into Mitaka City from another municipality or from overseas, please submit notification of moving in (“Tennyu Todoke”) within 14 days of living at your new address. These procedures cannot be completed unless you have completely moved in.

Required Documents:

1.The residence card, special permanent resident certificate, or foreign resident registration card, which is considered equivalent to these—and if you are moving in from overseas, your passport with the stamp imprint saying “Residence card to be issued at a later date” is acceptable—of all foreign residents moving in.
2.If you are moving from another municipality within Japan, a certificate of moving out (“Tenshutsu Shomeisho”) issued by the municipal office administering your previous address.

Other

 In any of the following cases, documentation certifying the family relationship with the head of the household is required; if the document is written in a foreign language, a Japanese translation is also required.
•If you are moving in from overseas
•If a new member is joining the family household
•If the family relationship is not written on the certificate of moving

Moving within Mitaka City

If you are moving within Mitaka City, please submit notification of moving (“Tenkyo Todoke”) within 14 days after moving to your new address. These procedures cannot be completed unless you have completely moved in.

Required Documents:

1.The residence card, special permanent resident certificate or foreign resident registration card, which is considered equivalent to the first two of all foreign residents moving.
2.If a new member is joining the family household, documentation certifying the family relationship with the head of the household is required. If the document is written in a foreign language, a Japanese translation is also required.

Moving Out of Mitaka City

 If you are planning to move to another municipality or to a foreign country, a notification can be completed from 14 days before your scheduled moving date.
If you are planning to move to another municipality, you will be issued a certificate of moving out upon submission of notification of moving out. This certificate is a required document when completing notification of moving in at the municipal office administering your new address. If you are planning to move to a foreign country, you will not be issued a certificate of moving out.
Note: These procedures can also be completed by mail. Please mail the following required documents to the Mitaka Municipal Office.
1.Required information form: This form can be downloaded via the Mitaka City website.
(With the title, “Notification of Moving Out,” please write your address in Mitaka City, the new address where you plan to move, the date you plan to move (or moved), and the full names, dates of birth, and daytime telephone numbers of all persons moving out.
2.A return envelope. Please write your name and address and affix ¥80 postage. This is not required for those who are moving to a foreign country.
Note: The address on the envelope should be either your address in Mitaka City or the new address where you plan to move.
3.A copy of some form of personal identification—such as your residence card or special permanent resident certificate, or your foreign resident registration card, which is considered equivalent to these two, and driver’s license or passport
